We know your time with loved ones is precious. Prioritizing your health today helps you stay present and ready to enjoy every moment that matters. That’s why completing your Annual Wellness Visit with your primary care doctor is so important. This appointment allows you and your doctor to talk about your needs and plan ways to stay healthy. Your Medicare Advantage plan includes one Annual Wellness Visit per calendar year. Plus, you can earn a reward just for completing it!

What should I expect at my Annual Wellness Visit?

An Annual Wellness Visit is not the same as a regular physical exam. It is a one-on-one preventive check-in with your doctor to:

• Complete a Health Risk Assessment
• Review your medical and family history
• Create/update your list of healthcare providers and medications
• Schedule vaccinations and preventive screenings
• Discuss your mental health, sleep and well-being
• Take care of any health concerns and answer your questions

Is it the same as an Annual Physical?

No, an Annual Wellness Visit is different from an Annual Physical.

What should I talk about at my Annual Wellness Visit?

Make the most of your visit by bringing questions and using this time with your doctor to discuss:

• Ongoing health conditions – Get help managing conditions like high blood pressure or diabetes.
• Medications – Review what you take and ask about refills and home delivery options.
• Vaccinations – Find out which vaccines you may need and when to get them.
• Mental health – Share how you're coping with stress, loneliness, anxiety or depression and get support if you need it.
• Staying active – Create an activity plan based on what you enjoy.
• Bladder health – Talk about any concerns and treatment options.
• Preventing falls – Learn about ways to reduce your risk with better strength, balance and a safer home.
• Family health history – Go over family health conditions that may affect you.

How do I schedule an Annual Wellness Visit and earn a reward?

• Step 1: Sign in to the HAP Member Portal and go to My Health & Wellbeing and then Healthy Living Rewards to make sure you are enrolled in our rewards program.
• Step 2: Call your primary care doctor and schedule your Annual Wellness Visit before Dec. 31, 2026.
• Step 3: Attend your visit and talk through your health questions with your doctor.
• Step 4: After your visit, we will process your claim and then your reward will be loaded onto your HAP flex card.
